SMS Consent & Opt-In Proof

What this page is for

This page documents how end users consent to receive text messages from Aesir Labs, LLC d/b/a ForthWrite as part of our consulting offer, so carriers and compliance reviewers can independently verify the opt-in flow without needing an account or login.

Where consent is collected

The public consulting practice is described at /consulting. That page can book a call without SMS. SMS consent is collected only on the consulting contact form used in our booking funnel.

The live public form is https://form.typeform.com/to/Eu7ihAB0. It collects first name, mobile phone number, and email. Entering a mobile number on this form is consent for appointment confirmations and reminders. The first screen is those three fields only. Marketing texts use a separate, optional, unchecked box on the next step. That marketing box is not required to submit the rest of the form or to book a consultation by another means. A phone number collected on any other ForthWrite page is not SMS consent.

All opt-in methods

This campaign uses one opt-in method:

• Website form: the person opens https://www.forthwrite.ai/sms-consent or https://form.typeform.com/to/Eu7ihAB0, enters their mobile number, and may check the optional marketing box. The same form may also be embedded on the consulting funnel.

We do not offer keyword opt-in, paper forms, verbal opt-in, QR codes, chat-widget opt-in, or any other method. We do not text purchased lists. We send appointment confirmations and reminders to people who enter a mobile number on this form. We send marketing texts only to people who checked the marketing box.

Consent language shown on the form

The booking form first screen is first name, phone, and email. Appointment terms are stated on this page. Marketing texts use a separate, optional, unchecked box. The appointment disclosure is:

“By entering your mobile number you agree to receive appointment confirmations and reminders from Aesir Labs, LLC d/b/a ForthWrite. Message frequency varies. Message and data rates may apply. Reply STOP to unsubscribe, HELP for help. Privacy, Terms, and SMS details: forthwrite.ai/sms-consent. 18+.”

The exact disclosure next to the optional marketing box is:

“Yes, text me about the consulting offer I asked for. Optional, not required to book. Recurring. Msg and data rates may apply. Reply STOP to opt out, HELP for help. Privacy, Terms, and SMS details: forthwrite.ai/sms-consent. 18+. A phone number alone is not consent.”

Message program details

• Program: Appointment texts are confirmations and reminders for the booked call. Marketing texts are about the consulting offer the recipient asked for, and they require the optional box.
• Message frequency: Varies based on your scheduled appointment and any follow-up about that offer.
• Cost: Message and data rates may apply.
• Opt-out: Reply STOP at any time.
• Help: Reply HELP for assistance.
• Age: You must be 18 years of age or older to opt in.
• Carriers are not liable for delayed or undelivered messages.
